Merck’s Sac-TMT: What One Global Phase 3 Trial Changes

MRK

Executive Summary

TroFuse-005 provides survival evidence for sac-TMT after platinum chemotherapy and immunotherapy. The results strengthen a specific product opportunity; the broader growth case still depends on further trials, regulatory decisions and adoption.

For Merck, sacituzumab tirumotecan has always represented a bet larger than a single clinical trial. Developed by Kelun-Biotech and licensed to Merck for exclusive development, manufacturing and commercialization outside Greater China, the TROP2-directed antibody-drug conjugate is central to a development program spanning multiple cancers and treatment settings. [3]

One important part of that investment thesis now has stronger support: sac-TMT can extend survival in a defined patient population.

Merck announced in May that the Phase 3 TroFuse-005 trial had met its primary endpoints. The detailed results disclosed at ESMO on October 26 allow investors to assess the size of the benefit and the treatment burden accompanying it. [1, 2]

What did this trial establish?

TroFuse-005 was a global, randomized, open-label Phase 3 study involving 776 patients with advanced or recurrent endometrial cancer previously treated with platinum-based chemotherapy and a PD-1 or PD-L1 inhibitor. It compared sac-TMT with the physician’s choice of doxorubicin or paclitaxel.

The dual primary endpoints were overall survival and progression-free survival assessed by blinded independent central review. [1]

The principal results were:

  • Median overall survival: 19.0 months with sac-TMT versus 12.1 months with chemotherapy. The hazard ratio was 0.68, with a 95% confidence interval of 0.55–0.84.
  • Median progression-free survival: 7.2 months versus 3.9 months. The hazard ratio was 0.59, with a 95% confidence interval of 0.50–0.70.
  • Grade 3 or higher treatment-related adverse events: 54.4% versus 41.4%. [1]

Both primary endpoints achieved statistical significance. The findings support improvements in disease control and survival.

The 6.9-month difference in median overall survival describes a difference between the trial groups’ survival distributions. It does not mean that every patient gains the same amount of time.

Safety also belongs in this assessment. The adverse-event figures above specifically describe treatment-related events. The higher frequency of Grade 3 or worse events means that supportive care, monitoring and dose management need to be considered when assessing the treatment’s practical value.

What does a global randomized trial add to a licensed asset?

For Merck, the study provides two forms of support.

First, it increases confidence in the clinical value of this particular indication. Randomization, a defined prior-treatment history and an overall survival benefit give the product opportunity a more concrete evidence base.

Second, it turns part of the global development strategy into a testable result. A licensing agreement provides development rights. Those rights acquire value as evidence accumulates in the relevant patients, against relevant comparators and for the regulatory pathway being pursued.

The scope of the conclusion still matters. TroFuse-005 established a relative benefit in previously treated endometrial cancer. It did not directly establish efficacy in other cancers, earlier treatment settings or against other ADCs.

Confidence in related studies can change after a result like this. The appropriate size of that update depends on how much biology, patient selection and treatment strategy those studies actually share.

Why does this matter for MRK?

Merck reported approximately $16.6 billion in worldwide sales for the second quarter of 2026. Keytruda and Keytruda Qlex together generated approximately $8.4 billion, or about half of the total. [4]

That revenue concentration helps explain why investors closely follow the company’s efforts to establish additional sources of growth. A survival-positive sac-TMT trial adds credibility to one of those paths.

The value of a single indication, however, remains several steps away from a product franchise capable of supporting company-wide growth. Merck’s October announcement described 18 ongoing global Phase 3 studies in the TroFuse program. [1]

Such a broad program represents opportunities and continuing financial commitments. Each new cancer, treatment setting or combination requires another assessment of efficacy, safety, the comparator and likely adoption.

A disciplined valuation update starts with the indication supported by the new evidence, then revisits other programs according to how transferable the findings are. Multiplying one successful trial across an entire development portfolio risks treating unproven opportunities as established returns.

What remains between evidence and commercial returns?

According to Merck’s October 26 announcement, the FDA has accepted the relevant biologics license application for priority review, with a target action date of June 15, 2027. [1]

That makes the near-term regulatory pathway more specific. The eventual label’s patient population, prior-treatment conditions and any testing requirements will help determine the addressable market.

Adoption will then depend on clinical use, patients’ ability to remain on therapy, reimbursement and treatment sequencing as competitors evolve. These are separate assumptions from the trial’s efficacy result.

Investors can work through the commercial opportunity in stages: how many patients qualify, how many receive the drug, how long they remain on treatment and what net revenue and profit Merck can ultimately earn. Its licensed rights provide access to the commercial opportunity in the relevant territories. Clinical success supplies necessary evidence for pursuing that opportunity. [3]

The new data justify greater confidence in specific parts of the investment case. Whether sac-TMT becomes a major growth pillar for Merck still depends on subsequent indication results, regulatory decisions and use after launch.

TroFuse-005 establishes a survival benefit in a defined patient population. The larger global product story still has to be delivered, indication by indication.
